While you can request an appointment date and time please understand that we are often unable to confirm because of the Doctor's surgery schedule. One of our office staff will call you to set up and confirm your appointment when Dr. Fox is scheduled to be in the office.

Typically, Dr. Fox sees new patients in the office on Wednesdays.

Once you hit submit, you will be taken to a page where you can download a "Pre-Visit" form. Please complete this form prior to your appointment and bring it with you when you check in at our reception desk.
